Welcome to the Union Middle School Athletics page!
All athletes are required to have a current sports physical and concussion form on file at the school, in order to participate in sports. The new updated forms are listed below. You may also contact the school to get a copy. You will need to make sure that all parent and student sections are filled out and the signature is completed BEFORE the physical can be accepted. Every year, we offer Sports Physicals at the school with Dr. Huff. They will be done right after school on August 21, 2024. The cost will be $10. If you need to confirm the date on your current physical on file you may call the school.
6th graders are eligible to play all sports except for football.
The sports offered for each season are listed below:

ELIGIBILITY
Middle school students have eligibility requirements to play sports. These requirements can be found on page 43 of the 2024-2025 student handbook under Athletics - Middle school eligibility requirements.
Student must have:
Satisfactory conduct - behavior matters
Math and Language Arts must be passed in addition to one less than the number of core classes the semester before. Students must also pass Health/PE.
Be present at school the day of the game in order to play (emergency situations can be justified by the principal).
Have the signed consent of his or her parents on file.
Have a current physical completed by a doctor stating he or she is in good physical health.
Have insurance.
Meet local promotion standards in order to be eligible to represent a school.
Must have a signed concussion form on file.
Must meet SCS attendance requirements.
All athletes and participants must be listed on the official eligibility list and must use county transportation to go to and from athletic events unless they are released to the parent or guardian with a signed release.
