Project AWARE Family Survey

Our Project AWARE school program needs your input to evaluate how well Project AWARE is accomplishing its goal to develop a plan of activities, services, and strategies for connecting youth and families to mental health services.

We are conducting an anonymous online survey that takes approximately 15 minutes to complete. Please take a few minutes to provide your input in helping Sampson County Schools support mental health services in our county. 

Included is a link to a survey about your child’s school. We are asking you to complete the survey to help us learn what parents and guardians think about our schools and services. Your thoughts and opinions are very important. Through this survey, we will use your experiences and views to help us build on our strengths and work to make improvements. 

The survey will take about 15 minutes to complete and can be accessed by desktop, laptop, or mobile device. Please complete this survey by March 31, 2023. If you have more than one child in Sampson County Schools, please complete the survey with your oldest child’s experiences in mind. There is no identifying information on the survey, so no one will know how you answered any of the questions individually. 

This survey is supported by North Carolina’s Department of Public Instruction. Please see the attached letter of endorsement.
